The Annual Plan is an optional membership offered through Docnote that covers uninsured services provided by your primary care provider. Instead of paying per request, an Annual Plan lets you access a range of services for one recurring fee — billed monthly or yearly.

  • Couple
  • Family
  • Senior
  • Senior Couple

How pricing works:

Pricing for the Annual Plan is set by your primary care provider — not by Docnote. Rates may vary depending on the plan type and your provider. Contact your primary care provider directly for specific pricing information.

Payment method:

Credit card is the only accepted payment method for the Annual Plan.

💡 Good to know: Enrolling in an Annual Plan does not affect the quality of care you receive. Your primary care provider provides the same standard of care regardless of whether you are on a plan.
