How to Create a Docnote Account

Learn how to sign up for Docnote in a few simple steps. You'll need your health card number, a valid email, and a phone number to get started.

Creating a Docnote account takes just a few minutes. Before you begin, make sure you are 18 years of age or older and have your health card number handy (version code not required).

To create your account:

  1. Go to https://app.docnote.com/login or open the invite link sent by your primary care provider.
  2. Enter your full name, email address, date of birth, health card number (version code not required), and phone number.
  3. Create a secure password.
  4. Read and accept the Terms and Conditions.
  5. Tap "Create Account" — a verification code will be sent to your mobile number or email address.
  6. Enter the verification code to confirm your account.
  7. Your account is now active and ready to use.

A note on phone numbers: A mobile number is strongly recommended. If you provide a landline, verification codes and messages from your primary care provider can only be delivered to your email instead.

⚠️ Important: You must be 18 or older to create a Docnote account. Children under 18 cannot register.
