How to Submit a Request or a Form in Docnote

Learn how to submit a service request or form to your primary care provider through your Docnote account in a few simple steps.

You can submit a request or form to your primary care provider directly through your Docnote account. Here's how to get started.

Steps to submit a request:

  1. Log in and go to your dashboard. Tap "Request a Service" — or navigate to Requests using the menu:
    • Phone or tablet: tap in the top left → select Requests
    • Computer: select Requests from the left-hand menu
  2. In the Request Notes field (required), describe what you are requesting in as much detail as possible. Include relevant dates and any medical details your primary care provider may need to complete your request.
  3. Attach a file if you have a form or document related to your request (optional).
  4. If the completed request needs to go to a third party — such as a lawyer, employer, or insurance company — enter their name and fax number or email address (optional).
  5. Tap Submit. You'll receive an email confirmation and can track the status of your request at any time from within your Docnote account.

💡 Tip: The more detail you provide in the Request Notes field, the faster your primary care provider can process your request. Include dates, context, and any relevant medical information upfront.

⚠️ Note: Prescription renewals are not available at every primary care provider.

Request statuses — you'll be notified when yours changes:
 
Pending Verification — your primary care provider has received your request
In Progress — your primary care provider is working on it
Pending Payment — payment required. Pay online by credit card or in office with cash or cheque
Completed — done. Final document attached if applicable
